A palatal expander is a gentle, time-tested way to widen a narrow upper jaw, and it works especially well while a child is still growing. What Is a Palatal Expander?

A palatal expander is a custom appliance that fits across the roof of the mouth and gently widens the upper jaw over a few weeks, then holds the new width while the jaw stabilizes. In growing children, the two halves of the upper jaw have not fully fused, so the expander can guide them apart comfortably.

We use a sintered RPE (rapid palatal expander). It is precise and fits snugly without the spacers, or separators, that older expanders required, so there is one less step and less discomfort for your child. When a case also needs the back teeth moved, for example after an early baby tooth was lost or to help correct the bite, we use a Pendex appliance to gently distalize, or move back, those teeth and recover the space.

Who It Helps

Expanders help children and teens with a narrow upper jaw, which often shows up as a crossbite, as crowding from too little room, or as teeth that simply do not have space to come in straight. By creating width, an expander can reduce crowding and, in some cases, lower the need for extractions later. It can also create more room in the upper arch.

The Right Age for an Expander

Timing is everything with expansion. The ideal window is roughly age 7 to 12, while the upper jaw is still growing. At that age the seam running down the middle of the upper jaw, called the maxillary suture, is still soft and has not fused, so a gentle expander can guide the two halves apart comfortably. Once that suture fuses in the mid-teens, a traditional expander no longer works on its own. That is one reason the American Association of Orthodontists recommends a first orthodontic visit by age 7, a great time to check whether the upper jaw has enough width.

Expansion and Breathing

A narrow upper jaw can crowd more than just the teeth. The roof of the mouth is also the floor of the nose, so when the maxillary palate is very narrow it can limit the space available for breathing and affect the volume of the maxillary sinuses. By widening a narrow upper jaw, expansion can open up that space and, in some children, support easier nasal breathing. We look at the airway as part of the bigger picture and coordinate with your medical providers when a breathing concern may be involved.

What to Expect

The expander is activated in tiny increments, usually with a small key, which creates gentle pressure rather than pain. There may be mild pressure or a tingle after each turn, and a short adjustment period for talking and eating that most kids get past within days. After the active phase, the expander stays in for about six months so the jaw can stabilize in its new position, and braces often follow to align the teeth.

How long is it worn?

What is the best age for an expander?

Will my child still need braces?

Usually yes. The expander creates width, and braces then align the teeth into the corrected bite.
